Duh... What other outcome would one expect from a government created and protected monopoly that forces us at gunpoint to use only "government approved" medicine? Why would this monopoly be any different than any other monopoly? i.e. high prices and lousy service. there is no other charge but our copay as long as we use a participating provider (we have a ppo). sounds to me like maybe you and others need a class action suit?

Double charging is FRAUD. Co-insurance was not on the form signed when purchasing the insurance. There's no such thing. Sue them! Co-pay is the sum of everything they do for you. Not to pay to walk through the door. Fraud. The false coding is FRAUD. Them signing your name is insurance fraud. Dble charging is FRAUD. Most folks are idiots and will pay it. That's what they count on. Good work Dave. Spread this everywhere. It takes lawsuits to stop this crap.

Yes, insurers are looking for ways for patients to pay more and they pay less. Managed care usually includes deductibles, co-pays, and coinsurance. Coinsurance is when the patient pays 20% and insurance 80%, or other percentage such as 30%/70%, etc. then most plans have a maximum out of pocket, such as once you pay $10,000 the insurer then pays 100% of covered expenses determined by the contract a person has with their insurer. Then the other side of this is the medical establishment that overcharge because they know that a insurer will demand a reduced rate. Then even worse patients with no insurance or who are not in network with their doctor by law must pay the full price, not the negotiated discount rate.

Under icd10 billing you sign a consent for treatment where you agree to pay in full for anything the insurance does not deem medically necessary.Insurance companies deny every claim on first submission.icd10 codes book is 4 inches thick. One wrong digit equals denial of claim. Nightmare for patients, billing and healthcare providers. Most people are unaware of this new system. Anyone accepts insurance as payment must wait months to get paid, if they even get a fraction of the total bill. Then the patient is liable for the balance due to the consent form signed. The only winner here is the insurance company. They collect premiums and refuse to pay claims.
